Abby Care is an AI-native platform for family-led care, addressing America's growing care crisis by enabling family caregivers to deliver high-quality home care with clinical oversight and technology support. The company partners with health plans, providers, and community organizations to expand access to care and improve outcomes. You will lead strategic partnerships in Colorado's case management community, building and scaling relationships with case management agencies (CMAs) on the Front Range. Your mission is to help CMAs understand Abby Care's value proposition—how the platform enables caregivers to deliver better quality care and achieve better patient outcomes—and to drive referral volume through trust-based, consultative engagement. Key responsibilities: - Build and own relationships with case management agencies, managing the full lifecycle from outreach through referral management - Target and map key stakeholders and decision-makers at independent CMAs, developing deep relationships - Educate partners on program fit, referral processes, and why Abby Care delivers superior outcomes - Drive referral volume to meet ambitious growth targets; compensation is partially performance-based - Prioritize field-based activity: face-to-face meetings, community networking, and regular partner check-ins - Track partner interactions, account status, and referral metrics in CRM systems You are mission-driven, self-directed, and growth-minded. You excel at relationship-building grounded in trust and transparency. You find satisfaction in creating connections that benefit all stakeholders—case managers, families, and Abby Care. You understand the urgency of the care crisis and thrive in a fast-paced, impact-focused environment. This is a full-time remote role based in Colorado with anticipated travel to meet partners in the market. Requirements: - 4+ years of field-based relationship or business development experience, ideally within Colorado's CMA landscape - Deep understanding of Medicaid, disability services, or home-based care - Familiarity with how Colorado CMAs operate, how case managers think, and what they need to confidently place clients - Consultative approach that positions you as a trusted expert, not just another vendor
