Sierra is building an AI platform to help businesses deliver better, more human customer experiences. The healthcare team has already reached over 50% of U.S. families through partnerships with leading health plans, providers, and healthcare platforms. You will design and build core healthcare integration primitives that enable Sierra agents to safely connect with EHRs and other healthcare systems of record. In this role, you'll own production integrations with major healthcare systems including Epic, athenahealth, Oracle Health, eClinicalWorks, and MEDITECH. You'll work across multiple integration patterns including FHIR, HL7, APIs, and flat files. You'll build reusable platform capabilities for healthcare data access, write-back workflows, authentication, authorization, auditability, observability, testing, and deployment across customers. You'll partner closely with Agent Engineers to design and ship healthcare workflows such as appointment scheduling, patient intake, care navigation, CRM updates, clinical task routing, and other read/write agent actions. You'll serve as a technical SME with customers' EHR, IT, security, and operations teams, translating complex interoperability requirements into clear implementation plans and durable platform capabilities. You'll also help define Sierra's standards for safe healthcare agent actions, including validation, idempotency, permissions, human-in-the-loop review, escalation, and PHI/data privacy safeguards. You bring 8+ years of hands-on software engineering experience at or above Senior level, with deep experience shipping production healthcare integrations under HIPAA, security, privacy, and compliance constraints. You have strong technical fluency with healthcare interoperability standards including FHIR, HL7, OAuth/SMART-style authorization, patient identity, data mapping, audit logs, and EHR-specific APIs. You've built integrations with major EHRs or healthcare platforms and have strong product and platform judgment about what should become reusable primitives versus customer-specific solutions. You're comfortable working directly with customers while staying deeply hands-on in code. A CS degree or equivalent professional experience is required. Bonus experience includes building and scaling end-to-end production systems, building or deploying AI/LLM systems in production, founding or founding engineer experience, familiarity with eval frameworks, agent tooling, RAG pipelines, and prompt engineering, prior experience with React, TypeScript, or Go, and previous roles interfacing with customers or leading technical projects with external stakeholders.
