Regard is an AI-powered healthcare platform that uses proactive documentation to improve clinical care delivery. As a Senior Backend Software Engineer on the Diagnostics team, you'll build core services that analyze patient medical charts—labs, medications, vitals, procedures, prior notes, and history—to identify conditions and surface supporting clinical evidence. You'll own the backend architecture and services that power this capability, working across the full product lifecycle from ideation through deployment and iteration. Your responsibilities include designing medium-to-large projects often starting from ambiguous problem statements, balancing speed-to-market with user experience, business goals, and technical quality. You'll model clinical data, design schemas and service contracts that new product surfaces depend on, and work directly with clinicians to translate clinical requirements into extensible logic and tooling. As a technical leader, you'll mentor other developers, drive architectural decisions, and maintain code quality through proactive improvements and codebase enhancements. You'll participate in on-call support and production monitoring for your areas of responsibility. The tech stack includes Python, Go, PostgreSQL, Redis, Celery for backend; TypeScript and React for frontend; AWS, Docker, Kubernetes, and GitLab CI/CD for infrastructure; and OpenAI/Anthropic for AI capabilities. You'll work with clinical standards including FHIR, ICD-10, SNOMED, LOINC, and RxNorm. Minimum qualifications: BS in Computer Science or equivalent, 6+ years professional software development experience, proficiency in Python or Go with production-quality code, proven experience building data-intensive applications with strong schema design skills, ability to participate in on-call support, and practical experience with AI coding tools (Cursor, Claude Code, Codex). Preferred: exposure to clinical/healthcare data, experience with ranked/probabilistic systems, startup or high-growth environment experience, tools for non-engineer domain experts, cloud infrastructure and LLM integration experience, and mature enterprise SaaS product background. Hybrid role based in New York City metro area (2 days/week in office, with more frequent onboarding and monthly team onsite weeks). Relocation assistance provided.
