Regard is an AI-powered Proactive Documentation platform that transforms healthcare delivery by analyzing patient data in electronic health records (EHRs) to recommend diagnoses and surface clinical evidence. The company's mission is to bring world-class healthcare to everyone by reducing physician burden, improving care quality, and enhancing hospital finances. As a Senior Backend Software Engineer on the Diagnostics team, you will build core backend services that read and interpret patient medical charts—including labs, medications, vitals, procedures, prior notes, and history—to determine which conditions are present with supporting evidence. You'll own the full product lifecycle from ideation through deployment and iteration, designing critical backend services that power Regard's platform. Key responsibilities include collaborating with product, design, clinicians, and engineers to design medium-to-large projects from ambiguous problem statements; owning backend services and driving improvements in system design, observability, testing, and performance; modeling clinical data and designing schemas and service contracts for new product surfaces; working directly with clinicians to translate diagnostic requirements into extensible logic and tooling; mentoring and reviewing code from other backend developers; maintaining and improving codebase quality through proactive fixes and architectural improvements; and participating in on-call operational support. The tech stack includes Python, Go, PostgreSQL, Redis, Celery (backend); TypeScript and React (frontend); AWS, Docker, Kubernetes, GitLab CI/CD, and Datadog (infrastructure); OpenAI and Anthropic (AI); and clinical standards including FHIR, ICD-10, SNOMED, LOINC, and RxNorm. Minimum qualifications: BS in Computer Science or equivalent; 6+ years professional software development experience; proficiency in Python or Go with production-quality code; proven experience building data-intensive applications with strong schema design skills; ability to participate in on-call support; willingness to travel up to 4 weeks annually; and practical experience with AI coding tools (Cursor, Claude Code, Codex). Preferred qualifications include exposure to clinical or healthcare data (FHIR, EMR integrations, clinical terminologies), experience with ranked/scored/probabilistic systems, startup or high-growth environment experience, tools for non-engineer domain experts, cloud infrastructure and LLM integration experience, and mature enterprise SaaS product experience. The role is hybrid with an expectation of 2 days per week in-office, with more frequent in-office work during onboarding and monthly team onsite weeks. Relocation assistance is provided. Candidates must be authorized to work in the US without visa sponsorship and located in NYC metro, SF Bay Area, or LA.
