Evergreen Nephrology is transforming kidney care through a value-based, person-centered approach. As Program Manager of Chronic and Complex Patient Management (CCPM), you will lead a care management team delivering longitudinal, patient-centered care for individuals with complex chronic conditions including kidney disease, diabetes, heart failure, and comorbidities.
You will oversee day-to-day operations of the CCPM care management teams, ensuring efficient execution of proactive care strategies that improve patient outcomes. Your responsibilities include leveraging clinical and operational expertise to guide management of high-risk populations, ensure adherence to established care pathways, and reduce complications, avoidable hospital admissions, and overall healthcare utilization.
You will be accountable for CCPM program performance, driving results against key performance indicators (KPIs) and quality metrics. You'll support ongoing team development through coaching, training, and performance management to ensure delivery of high-quality, efficient, and patient-centered care aligned with Evergreen's standards.
This is a fully remote position requiring availability during 8:30am–5pm Central or Eastern time. You will provide collaborative, supportive leadership to a care management team focused on improving outcomes for patients with kidney disease and related chronic conditions.
