Salary: USD 150,009.6 - 190,216 / annual
WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) is seeking a Primary Care Provider (Nurse Practitioner or Physician Assistant) to deliver comprehensive medical services to seniors in their homes and communities. Working under the direction of the Medical Director, you will collaborate with physicians and an interdisciplinary team (IDT) to complete assessments, develop care plans, and educate participants and families.
Key responsibilities include:
- Provide continuous medical coverage and perform comprehensive in-person assessments (initial, semi-annual, and unscheduled) for elderly participants
- Collaborate with physicians and IDT members to develop individualized participant care plans and ensure effective care coordination
- Arrange discharge planning from acute or long-term care settings and maintain participant care in skilled nursing facilities and residences with regular visits as needed
- Review outcomes and utilization data, including hospitalizations and specialist use, to establish best practices and policy improvements
- Participate in rotating night and weekend on-call coverage

Requirements:
- Graduation from an accredited school of nursing
- Current Nurse Practitioner or Physician Assistant certification with prescriptive authority
- Active RN state license
- Minimum 2 years of experience working with frail or elderly populations
- Current knowledge of chronic care, geriatric issues, and best practices
- Reliable transportation
- Willingness to participate in rotating night and weekend call
- Preferred: experience caring for seniors with complex care needs (Medicare/Medicaid population)
