Salary: USD 250,000 - 325,000 / annual

Daymark Health is a value-based oncology company redefining cancer care through a comprehensive, personalized platform combining dedicated care navigation, symptom-focused support, behavioral health, and social resources with evidence-based interventions and a hybrid in-person + virtual care model. As Primary Care Physician, Supportive Oncology, you will be a clinical lead for Daymark's expansion into New England, combining direct patient care with clinical oversight and serving as a visible leader in provider partnerships. You will operate as a player-coach: delivering direct care while shaping processes that enable partnerships to thrive. Key responsibilities: - Provide direct patient care across Massachusetts and Rhode Island markets, including comprehensive initial visits for newly enrolled patients, management of complex and high-acuity cases, post-discharge transitions, and clinical escalations. Conduct assessments, place orders, prescribe medications, and document in the EHR. - Help build the care model by partnering with regional leadership to refine SOPs, care pathways, and risk stratification approaches. Contribute clinical expertise to the design of new programs and workflows as the company grows. - Represent Daymark clinically with partners including oncology practices, primary care physicians, health systems, ACOs, and health plans. Bring clinical credibility to conversations, ensure Daymark meets clinical and quality obligations in each partnership, and support quality gap closure for patients in care. You will see patients virtually, partner with Nurse Practitioners, set documentation and communication standards, and serve as a steward of Daymark's clinical program. REQUIREMENTS: - Board-certified physician (MD or DO) with residency training in internal medicine, family medicine, or geriatric medicine - Able and willing to enroll in Medicare under Daymark promptly after start date - Active, unrestricted medical licenses in both Massachusetts and Rhode Island - Active DEA registration and eligibility to register in both states - Eligible to enroll as Medicare provider; not excluded, debarred, or sanctioned under any federal or state health care program - Minimum 5 years of practice experience including substantial direct patient care - Meaningful experience caring for patients with cancer, or in palliative care, hospice, or serious illness care (Board certification in Hospice and Palliative Medicine strongly preferred) - Experience supervising or collaborating with Nurse Practitioners; comfortable sharing accountability for clinical team performance - Experience in value-based or shared savings environments (ACO, Medicare risk arrangement, or similar); understanding of quality measurement, care gap closure, and documentation impact on program performance (direct home-based care experience preferred) - Comfortable caring for patients in their homes; willing to travel within Massachusetts and Rhode Island - Tech savvy; comfortable with virtual care via video and web-based platforms - Comfortable with ambiguity and building new programs - Strong communication skills with oncologists, primary care physicians, patients, and families - Valid driver's license required for in-home patient visits - Basic Life Support (BLS) certification required; compliance with Employee Vaccination and Testing policy required
