Aledade seeks a Practice Transformation Specialist (PTS) to serve as a direct interface between headquarters and healthcare provider partners in the Maryland/Delaware/DMV region. This role is central to Aledade's mission of improving patient care and reducing costs through population health initiatives.
As a PTS, you will be responsible for:
- Building and maintaining strong relationships with partner medical practices
- Understanding practice operations, workflows, and pain points
- Gathering insights into partner satisfaction, needs, and opportunities for improvement
- Supporting practices in implementing transformation initiatives aligned with population health goals
- Serving as the primary point of contact for assigned practices
- Identifying and communicating practice feedback to internal teams
- Driving adoption of Aledade's tools and methodologies
- Collaborating with clinical, operations, and product teams to support practice success
You are passionate about population health and genuinely motivated by improving patient outcomes while optimizing healthcare delivery costs. You excel at relationship-building, have strong communication skills, and can translate between clinical and operational perspectives. You understand healthcare delivery challenges and are committed to supporting practices through their transformation journey.
Candidates must be based in Maryland, Delaware, or the DC Metro area.
