Salary: USD 75,000 - 90,000 / annual
Groups Recover Together seeks a Population Health Coordinator with medical focus to support high-acuity member populations and those in transitional recovery periods. The role combines registry management, care coordination, and direct member engagement across multiple clinical disciplines.
Key responsibilities include maintaining accurate member registries, facilitating care team consultations, and tracking outcomes to drive quality improvement. You'll support complex care coordination for the most acute cases, assist in staff training on the Collaborative Care Model, and help design population-level quality initiatives.
Direct member care involves telephonic, SMS, and telemedicine outreach; screenings and brief therapeutic interventions; member education on co-occurring mental health, physical health, and substance abuse disorders; and transitional support for high-acuity members entering recovery. You'll coordinate treatment plan changes with clinical and medical providers, escalate crises appropriately, and support safety planning.
The medical focus specialization emphasizes medication management support (adherence monitoring, side effects, treatment effectiveness), creation of low-barrier educational materials for members and providers, review of medical data and documentation, and medical care coordination.
You'll also provide administrative and technical support on pilot projects as delegated by leadership. While the primary focus is medical, you're expected to contribute across mental health and social care areas as needed.
Required qualifications: strong understanding of care coordination and case management, knowledge of healthcare regulations and HIPAA, excellent communication across dispersed teams and external audiences, proficiency with telehealth platforms and in-person settings, ability to prioritize and adapt to rapid change, problem-solving skills, capacity to build trust with diverse populations, and proficiency with EMRs and Microsoft Office/Google Suite (spreadsheet skills essential).
Schedule: Monday–Friday, 8am–5pm EST. Full-time, remote position.
