The Patient Engagement Associate II will work collaboratively within an engagement pod with shared accountability for enrolling and onboarding children and caregivers into Imagine Pediatrics' program. - Reaching out to healthcare professionals engaged with attributed patients and caregivers, including durable medical equipment (DME) companies, home health organizations, pharmacies, pediatricians, and similar partners
- Leveraging relationships with healthcare professionals to support re-engagement of enrolled patients
- Handling inbound phone calls from caregivers, health plan service coordinators, and hospital discharge planners as part of the enrollment process
