Salary: USD 210,000 - 230,000 / annual

Form Health is a venture-backed virtual obesity medicine clinic delivering evidence-based obesity treatment through telemedicine. The company serves a rapidly growing market—obesity impacts over 40% of the US adult population, yet historically only ~1% received medical treatment. Form Health combines physician-led care teams with advanced practice professionals and registered dietitians, using a proprietary mobile app for video visits, text messaging, photo journaling, and digital data transmission. You will provide direct patient care focused on evidence-based obesity treatment and chronic weight management. Responsibilities include reviewing charts and conducting visits to ensure quality and continuity of care, providing treatment planning and medical management for complex cases, managing your own patient panel and/or collaborating with APPs and dietitians, prescribing non-controlled obesity medications, ensuring clinical documentation and protocols are followed, participating in team-based care initiatives, and attending departmental meetings to contribute to quality improvement. The role offers flexible scheduling: 32, 36, or 40 hours per week, with 80% clinical/patient-facing care and 20% administrative time. This is a fully remote W-2 position for US-based physicians. You will have the opportunity to mentor and collaborate with APPs, residents, and trainees in a clinical setting focused on high-quality, patient-centered care. REQUIREMENTS: - MD or DO degree - Current Board Certification in Obesity Medicine by the American Board of Obesity Medicine (ABOM) [REQUIRED] - Demonstrated clinical experience in obesity medicine and pharmacotherapy (older generation oral and newer injectable medications) - Active and unrestricted Medical State License in any US State, with eligibility to obtain additional state licenses (IMLC preferred but not required) [REQUIRED] - Minimum 2 years as an attending physician post-residency in Internal Medicine, Family Medicine, or Obesity Medicine, OR completed accredited Fellowship program in Obesity Medicine - Interest in mentoring, teaching, and collaborating with APPs and Registered Dietitians - Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting - Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity - Currently opted-in with Medicare [REQUIRED] - Committed to patient-centered, culturally competent care - Excellent communication and interpersonal skills - Experience providing synchronous patient care in telehealth setting (preferred) - Bilingual with medical fluency in Spanish (desired but not required)
