Daymark Health is a value-based oncology company redefining cancer care through a comprehensive, personalized platform that combines dedicated care navigation, symptom-focused support, behavioral health, and social resources with evidence-based interventions and a hybrid in-person + virtual care model.
As a National Virtual Oncology Nurse Navigator, you will be a key member of an integrated Care Team including a Medical Director, Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will provide triage, case management, and care navigation for oncology patients, many with additional comorbidities, working entirely through virtual platforms to improve patients' quality of life.
Key responsibilities include:
• Clinical Support: Provide consultative support over phone, email, or video; handle urgent clinical escalations as needed.
• Symptom Triage: Triage responses to Symptom Surveys and inbound reported symptoms, escalating to Nurse Practitioners when appropriate.
• Care Navigation: Proactively identify and address patient needs including care coordination, nutritional support, discharge planning, and provider referrals.
• Patient Education: Help new patients understand their diagnosis, Daymark's care offerings, and next steps.
• Relationship Building: Establish strong, trusting relationships with patients and care networks through empathy and active listening.
• Integrated Care Coordination: Collaborate with clinical team members (Nurse Practitioners, Medical Directors, Social Workers) and non-clinical Health Partners to address social determinants of health such as food security and transportation.
• Operational Excellence: Adhere to Daymark policies, procedures, and documentation standards; work with Clinical Operations to optimize systems and technology.
This is a full-time, remote position with hours Monday–Friday, 8:30 AM–5 PM in your respective timezone. You will support patients throughout the oncology care journey, from screening to survivorship or end-of-life care.

• Passion for helping patients understand the benefits of palliative care
• Oncology-related certifications strongly preferred or to be obtained within 2 years: Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)

• Bias to action; ability to identify and prioritize needs and act on urgent matters immediately
• Patient-centric approach with deep commitment to Daymark's mission
• Excellent organizational and multitasking skills in fast-paced environments
• Effective communication and listening skills
• Comfort with change and ambiguity; proven success in dynamic environments
• Basic Life Support (BLS) certification required; compliance with Daymark's Employee Vaccination and Testing policy required by start date
