Salary: USD 121,000 - 155,000 / annual
Devoted Health is seeking an experienced Advanced Practice Provider (NP/PA) to step into a leadership role as Manager of the Transitions of Care (TOC) Program. This is a hybrid clinical and management position where you will oversee the clinical and operational performance of APPs in the TOC program while continuing to provide direct patient care.
You will report to the Medical Director of the TOC program and join a collaborative leadership team including the Medical Director, APP Managers, Operations Leads, Nurse Director, and Staff Supervisors. Your dual focus will be split approximately 40% clinical care and 60% management responsibilities, though this balance will fluctuate based on team size.
Clinical Responsibilities (40%):
- Provide excellent care to patients in the 30-day post-discharge period
- Identify and fill gaps in care for post-discharge patients
- Escalate symptomatic patients through appropriate pathways
- Manage acute/chronic disease exacerbations (CHF, diabetic emergencies, COPD, hypertensive emergencies)
- Reduce preventable readmissions through proactive intervention
Management Responsibilities (60%):
- Oversee clinical performance and accountability of TOC APPs
- Conduct performance management to ensure team meets required targets
- Develop, track, and manage clinical and operational KPIs
- Identify improvement opportunities and implement strategies to achieve metrics
- Manage, empower, inspire, and align the growing team of TOC APPs
- Conduct bi-weekly one-on-one meetings with direct reports
- Set personalized goals and review individual performance
- Collaborate with program medical director and peer APP managers on processes and workflows
- Assist with recruitment, hiring, and onboarding of new TOC APPs
- Develop clinical processes and policies for the program
- Monitor clinical resource allocation and execute changes to meet visit needs
- Manage scheduling, time-off requests, and schedule density
- Oversee onboarding guidance for new providers in clinical and process development
- Serve as liaison to TOC leadership on status, issues, and opportunities in the APP organization
The role is fully remote and requires working Monday-Friday, 9:30am-6:00pm EST. Requirements:
- 5+ years working in clinical practice as an Advanced Practice Provider (NP/PA)
- At least 1 year of experience seeing post-discharge patients as an APP (NP/PA)
- 2+ years of previous management or team leadership experience
- Active and clear RN and APRN license or Physician Assistant license in home state
- National APRN certification and/or ability to get licensed in multiple states (required at hire, must be maintained)
- Ability to work Monday-Friday, 9:30am-6:00pm EST
