The Intake Coordinator plays a key role in facilitating the intake process for prospective PACE program participants. This role coordinates all intake activities—including home visits, assessments, and center tours—while partnering with prospective participants, caregivers, and cross-functional teams to ensure a seamless, participant-centered experience.
Core responsibilities include:
- Coordinating the end-to-end intake process by partnering with cross-functional teams to support timely enrollment
- Scheduling and coordinating home visits, center tours, assessments, and follow-up appointments
- Maintaining proactive communication with Outreach & Enrollment and clinical pre-enrollment teams
- Partnering with clinical teams to identify, coordinate, and facilitate additional assessments for participant eligibility
- Delivering a welcoming experience by guiding prospective participants and caregivers through center tours and educating them on the PACE model
- Maintaining accurate participant information and intake documentation in CRM, EMR, and organizational systems
- Tracking participant requirements throughout each intake stage to ensure timely completion of documentation
- Supporting high-quality participant handoffs from intake through enrollment into the Interdisciplinary Team (IDT)
- Supporting community events, enrollment conferences, and organizational initiatives
- Maintaining compliance with federal, state, and organizational regulatory requirements
The role requires strong organizational skills, the ability to manage multiple priorities simultaneously, and excellent communication with diverse stakeholders. The Intake Coordinator will work independently while collaborating across teams to achieve shared outcomes in a fast-paced, evolving healthcare environment.
**REQUIREMENTS:**
- Bachelor's degree in Healthcare Administration, Business Administration, Marketing, or related field, or equivalent combination of education and relevant experience
- Minimum 1 year of experience in healthcare, enrollment, participant services, care coordination, customer service, or similar role supporting participant engagement or enrollment processes
- Demonstrated ability to coordinate multiple priorities while managing participant information, scheduling, and documentation with strong attention to detail and timely follow-through
- Experience with Medicare, Medicaid, state and county eligibility programs, or other healthcare eligibility and enrollment processes (preferred)
- Strong communication and collaboration skills to effectively engage prospective participants, caregivers, and cross-functional teams
- Ability to work independently while collaborating across teams
- Proficiency with Microsoft Office and ability to quickly learn CRM, EMR, and other organizational systems
- Valid driver's license, reliable personal transportation, and automobile insurance meeting state requirements; ability to travel throughout service area
- Active CPR/BLS certification or ability to obtain within 30 days of hire
- Meets all participant-facing health requirements, including TB screening and required immunizations
