Habitat Health is a healthcare company that delivers comprehensive care to older adults through the Program of All-Inclusive Care for the Elderly (PACE). The Health Plan Associate – Program Management role is central to coordinating and monitoring cross-functional projects spanning claims, authorizations, pharmacy (Part D), provider networks, eligibility/census management, and member services. This position focuses on project execution, risk identification, and operational readiness to ensure key initiatives, regulatory deliverables, and performance improvements remain on track.
Key responsibilities include:
- Leading cross-functional healthcare projects and operational process improvements for the Health Plan function
- Driving initiatives across multiple domains such as vendor implementations, regulatory audits, process standardization, and data integrity efforts
- Maintaining end-to-end work plans for Health Plan operations including enrollment, authorizations, claims, pharmacy, and network performance
- Identifying risks, dependencies, and inefficiencies in operations
- Coordinating with stakeholders across claims, member services, provider networks, and other departments
- Tracking project timelines, deliverables, and milestones
- Supporting operational readiness for new Health Plan products and regulatory compliance
- Documenting processes and maintaining project documentation
The role requires strong organizational skills, attention to detail, and the ability to manage complex, multi-stakeholder initiatives in a healthcare environment. The successful candidate will help scale Habitat Health's PACE model as the company expands to serve growing populations of older adults.
