Form Health is a venture-backed virtual obesity medicine clinic founded in 2019, delivering evidence-based treatment through telemedicine. The company combines board-certified physicians, advanced practice professionals, and registered dietitians with proprietary mobile app technology to serve patients across the US. As an Eligibility Specialist, you will be the critical first touchpoint ensuring patients can safely and efficiently access the program.
You will review patient records and clinical documentation to determine eligibility for Form Health's obesity medicine program, manage the collection of medical records from primary care providers and specialists while maintaining HIPAA compliance, and maintain clear communication with patients throughout the eligibility process. This role reports to the Manager of Eligibility and requires meticulous attention to detail, strong organizational skills, and the ability to juggle multiple patient cases simultaneously.
The ideal candidate has at least one year of experience in primary care, endocrinology, or urgent care settings with demonstrated expertise in medical record review and collection. You should be proficient with electronic medical record (EMR) systems, have an active Medical Assistant Certification, and possess working knowledge of HIPAA regulations. Experience in remote care settings is preferred. You'll need exceptional written and verbal communication skills to correspond professionally with patients, clinicians, external providers, and internal colleagues, and the ability to work both independently and collaboratively. This is a full-time remote position available anywhere in the USA.
