Claim Health is an AI-powered Revenue Platform for Post-Acute Care, serving home health, skilled nursing, home care, and hospice providers. The company is YC-backed and early-stage, addressing fragmentation in post-acute care operations by unifying data, automating workflows, and enabling real-time decision-making across revenue-critical functions including referral intake, eligibility verification, insurance mapping, prior authorization, and admit decisions.
You will be Claim Health's first Customer Success Manager, owning the post-launch partnership with home health and hospice agencies. You'll serve as a strategic partner to agency leadership—from intake directors to executives—building trusted, long-term relationships and advocating for each customer inside the company.
Key responsibilities include:
• Own Agency Partnerships: Serve as the strategic partner to agency leadership, build trusted relationships, and own renewals, expansion, and customer references.
• Drive Performance & Growth: Build individualized growth plans for each agency with clear goals to increase admissions and revenue. Run monthly and quarterly business reviews demonstrating impact and identifying expansion opportunities (new referral sources, automated workflows, new service lines, branch expansion).
• Shape the Product: Bring customer insights directly into product priorities, partner with engineering to ship features, and keep customers informed about roadmap developments.
• Build the Customer Success Function: Define how Claim Health executes customer success, including account health frameworks, business review cadences, and playbooks. Leverage AI and internal tooling to deliver high-touch experiences at scale.
You'll work closely with operations, product, and engineering teams. Your insights from customers will directly influence what the company builds next. Travel for onsite agency visits is expected.
REQUIREMENTS:
• 2–5 years of experience in one of the following:
- Customer success, account management, or implementation at a healthcare SaaS, RCM (Revenue Cycle Management), or early-stage startup, OR
- Revenue cycle or intake operations at a home health, hospice, or post-acute provider
• Demonstrated track record of growing accounts, with quantifiable results
• Analytical mindset with ability to pull, interpret, and present data to executives
• Strong rapport-building skills across diverse stakeholder groups (intake coordinators to agency executives)
• Clear, concise written and verbal communication
• Ability to thrive in fast-paced, ambiguous environments
• High ownership mentality and bias toward action
• Willingness to travel for onsite agency visits
NICE TO HAVE:
• Familiarity with home health and hospice revenue cycle (payer mix, eligibility, authorizations)
• Experience with EMRs (HCHB, KanTime, WellSky, Axxess)
• Comfort with data tools (SQL, Excel/Google Sheets, dashboards)
• Experience collaborating with product and engineering teams
• Habit of automating work via scripts or spreadsheets
