Lyra Health is a leading mental health care provider serving over 20 million people globally through employers and health plans. The company has delivered 15+ million therapy sessions and published 35+ peer-reviewed studies demonstrating that their AI-powered care model helps clients recover twice as fast and reduces overall healthcare claims costs by 26% annually.
As a Complex Care Specialist, you will manage a caseload of clients with complex, multi-need behavioral health issues. Working exclusively by phone and email, you will assess clinical needs, provide crisis support, facilitate engagement in treatment, and navigate clients into appropriate evidence-based facilities and programs. This is not a therapy role but rather a case management and care coordination position.
Key responsibilities include:
- Independently managing a caseload of complex clients with long-term case management support
- Conducting risk and needs assessments, safety planning, and de-escalation during crises
- Responding to in-the-moment crisis callers with evidence-based coping skills
- Using Lyra's proprietary matching software and facility databases to identify and connect clients to appropriate treatment options
- Administering and monitoring symptom measures (PHQ-9, GAD-7, etc.)
- Developing personalized care plans aligned with client needs and preferences
- Coordinating care with preferred facility network providers, health plans, and navigators
- Participating in multidisciplinary team meetings and case presentations
- Completing screenings for Lyra's DBT program
- Participating in on-call rotation for warm referrals
- Providing feedback to leadership for program improvement
You will work Monday–Friday, 9:00am–5:30pm PT, in a remote capacity. The role requires strong clinical judgment, motivational interviewing skills, knowledge of higher levels of care, and the ability to work independently while collaborating with a small clinical team.
Requirements:
- Master's degree in clinical social work or equivalent
- Independent clinical licensure required (LCSW, LMFT, LPC, LPCC, or equivalent); candidates without state licensure to independently deliver mental health services will not be considered
- Significant experience managing high-risk clinical situations (suicidality, mandated reporting, etc.)
- Experience navigating health plans and encouraging client engagement with escalating mental health needs
- Proficiency in case management and specialized care delivery for diverse client populations
- Knowledge of higher levels of care and community mental health resources
- Ability to work independently in a remote environment and collaborate effectively within a small team
- Familiarity with health plan procedures and medical necessity criteria
- Must be a full-time US resident; work cannot be completed from outside the United States
- Bilingual Spanish-speaking candidates strongly preferred but not required
