As a Clinical Pharmacist in Ambulatory Care, you will be embedded within primary care practices in the Rhode Island Primary Care Physicians Corporation (RIPCPC) ACO, serving as the medication expert on interdisciplinary care teams. Your primary focus will be supporting a pharmacist-led COPD population health program for high-risk patients, including transitions of care management, medication optimization, inhaler education, adherence support, and early exacerbation intervention.
Key responsibilities include: reconciling and optimizing medication regimens for safety, effectiveness, and adherence; providing evidence-based medication therapy recommendations in collaboration with providers; delivering chronic disease state management with emphasis on COPD guideline-directed therapy; identifying and engaging high-risk COPD patients through population health platforms; conducting transitions of care evaluations following acute care encounters; educating patients on medications, disease states, and self-management strategies; practicing within collaborative practice agreements; participating in quality improvement initiatives and population health program development; documenting patient encounters in the EHR; tracking and reporting program outcomes and quality metrics; and supporting additional ambulatory clinical pharmacy services as needed.
You will function as a medication and drug information resource for patients, providers, and care team members, working to optimize medication regimens, improve chronic disease outcomes, and reduce avoidable healthcare utilization. The role emphasizes evidence-based, patient-centered care delivered in collaboration with an interdisciplinary team focused on high-quality, cost-effective outcomes.
