Salary: USD 120,000 - 130,000 / annual

As a Bilingual Nurse Practitioner (Advanced Practice Provider), you will deliver remote telemedicine care to Twin Health members across multiple states. You'll work Monday–Friday with a flexible schedule (three shifts 11am–8pm PST and two shifts 9am–6pm PST) as part of a collaborative care team including health coaches, physicians, and the Chief Medical Officer. Key responsibilities include conducting comprehensive health assessments (reviewing labs, medical history, psychosocial factors), assessing and treating symptoms in collaboration with members' primary care providers, managing a high-risk population with attention to social determinants of health, prescribing medications with proper dosing guidance, maintaining accurate clinical records, and providing patient education aligned with Twin's diabetes reversal program. You'll use Twin's clinical platform for documentation and work to understand the Twin Model of Care. You'll also collaborate on service level agreements supporting financial, clinical, and quality outcomes, and deliver excellent customer service focused on managing multiple chronic conditions. Qualifications: - Active ARPN (Advanced Registered Practitioner Nurse) California state license; preferably licensed in multiple states - Bilingual fluency in English and Spanish (required) - 3–5 years of experience as an advanced practice provider in one or more of: family medicine, endocrinology, obesity medicine, adult medicine, chronic care management, or geriatric care - Clinical experience and demonstrated passion for working with diverse populations, including underserved groups - Experience managing patients remotely across different geographic areas and states - 5 or more state licenses with willingness to obtain additional licenses - Board certification in family or internal medicine - Proven advanced practice provider experience with sound medical knowledge - Ability to consult with patients via virtual communication channels - Excellent analytical, problem-solving, and communication skills - Patient and compassionate disposition; detail-oriented - Interest in working with underserved populations - Experience using technology and data to guide clinical decisions
