Imagine Pediatrics is a tech-enabled, pediatrician-led medical group delivering 24/7 virtual-first and in-home medical, behavioral, and social care for children with special healthcare needs. The company works alongside families, providers, and health plans to break down barriers to quality care for medically complex children.
As a Behavioral Health Care Coordinator, you will provide case management and clinical support for pediatric patients (up to 21 years old) enrolled in the behavioral health program, many recently discharged from hospitals. You'll work within an interdisciplinary team including pediatricians, nurses, and other healthcare professionals.
Key responsibilities include: monitoring high-risk patients and ensuring appropriate follow-up care; conducting biopsychosocial assessments to address behavioral, social, emotional, and systemic needs; creating and evaluating care plans that evolve with family needs; providing crisis intervention and de-escalation support; facilitating case management across complex systems (mental health, medical, social services); connecting families with validated external services and behavioral specialists; delivering psychoeducation on mental health diagnoses and treatment adherence; collaborating with external care teams (schools, specialists, child welfare agencies); and participating in interdisciplinary consultations to monitor patient progress.
The role is fully remote with a Monday-Friday schedule, with shift options of 8:00am-5:00pm or 10:30am-7:00pm. You'll represent Imagine Pediatrics to patients, families, providers, and the community while supporting families navigating the complexities of mental health diagnoses and co-occurring medical conditions.
