Hopscotch Primary Care is transforming healthcare access in rural America, addressing the gap where 20% of Americans live in rural communities but only 11% of physicians practice there. The company operates a value-based care model delivering high-quality primary care through clinics and community-based settings.
As Associate Outreach Leader, you will be a key member of the Outreach team, driving patient growth and community engagement across assigned clinics and territories in Asheville. This role combines community outreach, event coordination, grassroots relationship management, and prospective patient engagement to increase awareness of Hopscotch and connect eligible patients to care.
Key responsibilities include:
**Community Outreach & Relationship Building:** Build and maintain relationships with grassroots community organizations including senior centers, housing communities, faith-based organizations, nonprofits, and civic groups. Serve as a trusted brand ambassador, educating community members on Hopscotch services and the value-based care model. Maintain recurring touchpoints with community organizations for long-term engagement.
**Community Events & Open Houses:** Co-lead or independently support outreach events including health fairs, educational seminars, senior community visits, and clinic open houses. Handle venue preparation, materials coordination, staffing, setup, and follow-up. Engage prospective patients during events, capture information, and conduct timely follow-up to support conversion.
**Patient Engagement & Growth:** Actively identify and engage eligible Medicare and Value-Based Care patients through calls, texts, email, and in-person interactions. Assist with appointment scheduling and enrollment navigation. Conduct follow-up with prospective patients from outreach, events, and referrals to improve conversion and retention.
**Operational Excellence:** Track outreach activities and outcomes in Salesforce and other systems. Maintain outreach calendars, event timelines, and materials inventory. Collaborate with Outreach Leaders, clinic teams, marketing, and patient engagement teams to maximize community presence and support patient acquisition.
You will work closely with Outreach Leaders and internal teams in a collaborative, fast-paced, mission-driven environment. The role requires strong relationship-building skills, comfort engaging with seniors and community partners, excellent organizational abilities, and a proactive, solutions-oriented mindset.
