Chamber Cardio is rebuilding the cardiology system by partnering with independent cardiologists to lead population health efforts in their communities. The company equips physicians with technology, data, and operational tools to drive better patient outcomes through a blend of clinical expertise, thoughtful design, and modern operating platforms.
As Team Lead, Care Management, you will build and scale Chamber's cross-functional care team—Community Health Workers, Medical Assistants, Pharmacy Technicians, and RNs—who form the operational engine behind payor and network contracts. You'll drive member outreach, post-hospital follow-up, medication management support, and care coordination.
In this hands-on management role, you will own daily operations including team huddles, timecards, call quality monitoring, performance management, staffing decisions, and work assignments. You'll be the operational backbone the team depends on, partnering closely with leadership as programs and workflows evolve. You'll work cross-functionally with Clinical, Care Operations, and Product teams to keep execution aligned with strategy, reporting to the Associate Director, Care Management.
Key responsibilities include: facilitating daily huddles to prioritize tasks and surface blockers; monitoring real-time task queues to ensure work distribution and SLA adherence; serving as the first point of contact for staff questions and coverage gaps; overseeing patient voicemail management; ensuring same-day documentation completion for billable activities; conducting weekly 1:1 meetings with direct reports to review workload, call quality, documentation accuracy, and development needs; delivering specific, actionable feedback; identifying and coordinating targeted coaching on documentation standards, call handling, and program protocols; tracking attendance and schedule adherence; coordinating with contract agency account managers on staffing and performance issues; monitoring 5-10 recorded calls monthly for quality assurance; preparing weekly performance summaries with KPIs and escalations; identifying operational bottlenecks and recommending solutions; supporting contact rate optimization initiatives; reconciling contract invoices against timecards; and maintaining team rosters and scheduling documents.
In your first 90 days, you will onboard key Care Team roles to ensure proper structure and capacity; own team performance against expectations; own the call quality scorecard and address barriers; and identify and implement at least one operational improvement with measurable results.
REQUIREMENTS:
- 3+ years of experience supervising predominantly remote frontline teams in healthcare operations, care management, or care coordination
- Direct experience managing outbound member/patient outreach teams; understanding of contact rates, call quality, and high-volume outreach operations
- Experience with timecard management, attendance tracking, and performance coaching for teams of 6+ workers
- Proficiency with CRM and care management platforms (HubSpot, Salesforce, EHR systems, or similar)
- Strong written and verbal communication skills; ability to deliver direct, constructive feedback
- HIPAA knowledge and comfort handling protected health information
- Comfort operating in ambiguity, adapting quickly as programs evolve, and building in motion
PREFERRED:
- Prior experience building or scaling teams in a startup or high-growth environment
- Experience managing frontline staff contracted through a staffing agency
- Experience in value-based care, managed care, or population health settings
- Background as a Community Health Worker, Medical Assistant, or LPN who moved into a supervisory role
- Experience with Community Health Worker programs and CHW supervision best practices (motivational interviewing awareness, supportive supervision models)
- Experience with cardiology, cardiovascular care coordination, or chronic disease management
- Bilingual (English/Spanish) capability given member demographics
You must be based in the United States and have the ability to work ET/CT timezone. Periodic travel to practice sites or Chamber offices may be required.
