Salary: USD 210,000 - 250,000 / annual

Midi Health is the largest virtual care clinic for women in midlife navigating perimenopause, menopause, and other hormonal transitions. The company combines expert clinicians, evidence-based protocols, and a modern technology platform to deliver care historically underserved by the healthcare system. In this Staff Software Engineer role, you will set the technical direction for how Midi builds with AI across clinical workflows, patient experience, and operational intelligence. You'll own the architecture of AI/LLM systems, establish evaluation practices that ensure safety in a healthcare context, and define standards that enable every engineer at Midi to ship AI features confidently. Key responsibilities include: - Lead technical strategy for AI/LLM systems across Midi's product surfaces - Architect retrieval, orchestration, agentic, and evaluation systems that run reliably in production - Set standards for AI safety, evaluation, observability, and responsible rollout in a regulated healthcare environment - Partner with clinical, product, and executive leadership to shape the AI roadmap - Mentor senior and mid-level engineers into strong AI engineers and raise AI literacy across the organization - Evaluate new models, techniques, and tools, bringing the right ones into the team The company operates hybrid by design with two days per week in-office (Tuesday and Thursday) in either Palo Alto or San Francisco. Midi is AI-native in its engineering culture, expecting every engineer to be fluent with modern AI coding tools (Claude Code, Cursor, Copilot, etc.) and to actively push the frontier of how software is built. The team values low-ego, high-curiosity collaboration, outcome-oriented execution, and investment in each other's growth. Requirements: - 10+ years of software engineering experience with deep recent time leading production AI/LLM systems - Seasoned hands-on coder who still writes production Python regularly - Seasoned system designer for AI systems at scale (retrieval, agents, evaluation, latency, cost optimization) - Authority in AI engineering: teams and leaders defer to your judgment, and you can defend it rigorously - Leader at the AI frontier: you set how the team uses AI tooling and how the company builds AI products - Seasoned mentor with a track record of growing engineers into strong AI builders - Strong communicator with product, clinical, and executive stakeholders; thoughtful about risk and safety tradeoffs - Low-ego, curious, and humble Nice to have: - Healthcare, clinical NLP, or regulated AI deployment experience - Track record of shipping agentic or large-scale RAG systems in production
