Commure is building an AI Operating System for healthcare that spans the full care journey: ambient AI and dictation, intelligent agents automating workflows, and autonomous revenue cycle management. The company processes $25B+ in annual claims and supports 200+ million patient interactions across 500+ healthcare organizations. You'll join a small, fast-moving team building AI Voice Agents that handle incoming patient calls end-to-end: scheduling, rescheduling, cancellations, confirmations, FAQs, and intake. These agents integrate directly with EHR and telephony systems in real time to solve a critical problem—85% of patients who can't reach call centers never call back and go elsewhere. In this role, you'll design and build the backend systems powering these voice agents. You'll architect low-latency pipelines connecting telephony, ASR, LLMs, and TTS into a single conversational loop. You'll lead integrations with major healthcare systems (Epic, Cerner, athena, etc.) for scheduling, eligibility, and intake. You'll drive design decisions on APIs, data models, and event-driven workflows while mentoring other engineers. You'll partner across AI, product, and operations to ship features that patient access teams actually use, and you'll set the bar for latency, reliability, observability, and release readiness. The team ships daily, works directly alongside clinicians, and the gap between what you build and its impact on patient care is immediate. You'll have significant room to grow into harder problems on a high-performing team. REQUIREMENTS: - 4+ years of backend software experience, primarily in Python - Track record of designing and shipping production services, APIs, and data models that others build on - Intuition for how people interact with automated assistants through voice and text - Deep experience with async, event-driven, or real-time systems - Strong fundamentals in testing, LLM evaluation, observability, and reliability - Patient-centered mindset; design for the person on the other end of the line, not just metrics - Bias for action, ownership mindset, and judgment to know when to move fast and when to slow down
