Form Health is a venture-backed virtual obesity medicine clinic delivering evidence-based treatment through telemedicine. Founded in 2019, Form Health is scaling rapidly in the growing obesity medicine field.
The Provider Scheduling Coordinator will own day-to-day execution of provider calendar management, including time-off requests (DTO), shift swaps, and schedule adjustments. This role is primarily focused on internal provider calendar optimization rather than patient-facing scheduling, though may coordinate with patient scheduling teams during high-volume periods.
Key responsibilities include:
- Managing provider calendar changes while ensuring alignment with internal policies, coverage requirements, and capacity needs
- Monitoring schedules to identify gaps, overages, or misalignments based on clinical demand and Time to First Appointment SLAs
- Maintaining accurate scheduling records and auditing internal changes for consistency and compliance
- Serving as primary point of contact for providers on scheduling logistics
- Coordinating with clinical leadership to plan coverage during provider leave, onboarding, or offboarding
- Collaborating with Credentialing & Licensing and regional leaders to ensure calendar changes reflect licensure readiness and panel assignment requirements
- Working with Practice Operations to forecast resourcing needs during volume spikes or workforce changes
- Supporting transition of care workflows by updating schedules for new clinician assignments
- Recommending improvements to scheduling workflows and assisting with implementation of new tools and SOPs

Requirements:
- 2+ years of experience in scheduling for at least 15 clinicians, managing high-volume calendar requests, or working in healthcare operations/administrative coordination (telehealth startup experience preferred)
- Experience with scheduling platforms, EHR systems, or internal workflow tools (e.g., Jira, Asana, Salesforce)
- Excellent organizational and communication skills
- High attention to detail and ability to manage multiple priorities
- Proficiency in Google Sheets/Excel with ability to analyze and spot schedule gaps or utilization issues
