As an Enhanced Care Management (ECM) Program Manager based in the Inland Empire, you will lead an interdisciplinary team delivering compassionate, impactful care to populations with complex medical and social challenges. You will supervise a team including a Registered Nurse, Care Coordinator, and Community Health Worker, while also serving as the team's behavioral health resource.
Key responsibilities include:
- Supervise and mentor an interdisciplinary care team
- Maintain a personal caseload of patients with behavioral health needs and deliver high-quality direct care
- Conduct behavioral health screenings and assessments to inform individualized care plans developed in partnership with patients and the team
- Serve as the go-to resource for behavioral health questions, warm handoffs, and crisis response protocols
- Ensure patients presenting with suicidality, acute mental health concerns, or substance use disorder crises are appropriately triaged and connected to care
- Document all encounters accurately and timely in Care Director in compliance with IEHP ECM program requirements and CalAIM regulations
- Participate in community outreach and field-based work
- Contribute to program development and quality improvement initiatives
This is a hybrid role based in the Inland Empire with regular in-person presence at Chaparral Medical clinics (particularly Pomona and Rancho Cucamonga), community outreach in the field, and occasional travel to LA for all-hands meetings. Local candidates are strongly preferred.
Minimum education requirement: Master of Social Work (MSW) or Marriage and Family Therapy (MFT) degree. California licensure (LCSW/LMFT) is preferred. Must reside locally to the Inland Empire region.
