Chamber Cardio is rebuilding the cardiology system by partnering with independent cardiologists to lead population health efforts. The company combines clinical expertise, thoughtful design, and modern operating platforms to support physicians, patients, and payers, using AI tools and data to improve cardiovascular outcomes at scale.
As Program Manager, Care Operations, you will execute and scale Chamber's AI-enabled cardiovascular care programs, reporting to the Director of Program Operations & Infrastructure. This role bridges Clinical, Product, and Care Delivery Operations, translating clinical protocols into executable workflows and driving cross-functional coordination.
Key responsibilities include:
• Own end-to-end program execution from workflow documentation through go-live tracking to post-launch iteration for active care contracts and attributed populations.
• Coordinate across Clinical, Product, Engineering, and Care Delivery Operations to identify blockers, close gaps, and maintain program timelines and deliverables.
• Serve as the consistent point of contact for cross-functional stakeholders, ensuring alignment on priorities and progress.
• Translate clinical protocols and product capabilities into operational workflows that Care Delivery teams can execute consistently across markets.
• Build and maintain SOPs, playbooks, and training materials that enable reliable program execution.
• Identify workflow gaps, unclear handoffs, and missing infrastructure, taking ownership of solutions.
• Partner with Product and Engineering to integrate AI tools into care workflows, understanding where automation scales and where human intervention is needed.
• Support configuration, testing, and rollout of new tools and workflow changes.
• Track operational performance and surface opportunities to reduce manual burden through process improvements or tooling enhancements.
In your first 90 days, you will review and document workflows, staffing, and tooling for active care programs; build core operational infrastructure including SOPs and escalation paths; establish working relationships across Clinical, Product, and Care Delivery; and deliver a comprehensive status update on program execution with recommendations.
You bring 4-8 years of healthcare operations, program management, or adjacent experience, with preference for value-based care, population health, or managed care backgrounds. You demonstrate the ability to create structure in ambiguous environments, excel at process documentation that teams actually use, and have experience with care management platforms or AI-enabled healthcare tools. You are analytical, data-driven, a clear communicator, and passionate about improving outcomes for cardiology patients and providers.
