WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) is seeking a Primary Care Provider (Nurse Practitioner or Physician Assistant) to deliver comprehensive medical services to seniors in their homes and communities. This role emphasizes holistic, value-based care through an interdisciplinary team (IDT) approach.
You will work under the direction of the Medical Director, collaborating with physicians and IDT members to complete assessments, develop care plans, and educate participants and families. The role offers a smaller patient caseload compared to traditional primary care settings, enabling deeper long-term relationships and continuity of care.

- Providing continuous medical coverage and performing comprehensive in-person assessments (initial, semi-annual, and unscheduled)
- Collaborating with physicians and IDT members to develop participant care plans and ensure effective care coordination
- Arranging discharge planning from acute or long-term care settings
- Maintaining participant care in skilled nursing facilities and residences with regular visits as needed
- Reviewing outcomes and utilization data (hospitalizations, specialist use) to establish best practices and policy improvements
- Participating in rotating night and weekend on-call coverage
WelbeHealth emphasizes work-life balance with 17 days PTO, 12 observed holidays, 6 sick days, and CME reimbursement. The organization uses a collaborative IDT model to treat the whole person, not just symptoms.
Requirements:
- Graduation from an accredited school of nursing
- Current Nurse Practitioner or Physician Assistant certification with prescriptive authority
- Current RN state license
- Minimum 2 years of experience working with frail or elderly populations
- Current knowledge of chronic care, geriatric issues, and best practices
- Reliable transportation
- Willingness to participate in rotating night and weekend call
- Preferred: experience caring for seniors with complex care needs (Medicare/Medicaid population)
