Somatus is a leading outcomes-driven care provider supporting individuals with chronic conditions including cardio, kidney, metabolic, and other chronic illnesses. The Patient Health Advocate (PHA) role is a hybrid position based in Salt Lake City that serves high-needs Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) populations.
In this role, you will be the primary Somatus representative embedded within physician practices, establishing trust and building relationships with patients and provider teams. You'll spend time both in physician offices and visiting patients' homes, working under the guidance of practice physicians and nurse care managers.
Key responsibilities include:
- Conducting patient outreach (telephonic and in-person) to introduce the Somatus program, encourage enrollment, and engage patients
- Serving as the primary contact for provider practices regarding patient needs and care coordination
- Supporting care transitions and assisting patients during post-discharge periods to minimize readmissions
- Helping patients schedule appointments, access community resources, and navigate language/cultural barriers
- Documenting all activities in the care coordination platform and collaborating with interdisciplinary care teams
- Coaching patients using motivational interviewing techniques to support behavioral change and self-care goals
- Facilitating in-home telehealth visits and reinforcing clinical education
- Assisting with appointment scheduling, transportation coordination, and access to educational resources
You'll work as part of a multi-disciplinary team including physicians, registered nurses, and care managers. The role requires strong communication skills, cultural awareness, computer proficiency (MS Office, telecom platforms), and the ability to manage multiple priorities while supporting vulnerable patient populations.
