Somatus is a leading outcomes-driven care provider supporting individuals with chronic conditions including cardio, kidney, metabolic, and other chronic illnesses. The Patient Health Advocate (PHA) role is a hybrid position supporting high-needs Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) populations.
In this role, you will be the primary Somatus representative embedded within physician practices, responsible for establishing trust and building collaborative relationships with physicians, practice staff, and patients. You'll split time between remote work, physician office visits, and in-home patient visits.
Key responsibilities include:
- Conduct patient outreach (telephonic and in-person) to introduce the Somatus program, encourage enrollment, and engage patients
- Serve as the primary contact for provider practices regarding patient needs and care coordination
- Support care transitions, particularly for recently hospitalized patients, to minimize avoidable readmissions
- Assist patients with scheduling appointments, accessing community resources, and arranging transportation
- Coach patients using motivational interviewing techniques to support behavioral change and self-care goals
- Address language and cultural barriers to care management
- Document all activities in the care coordination platform
- Support interdisciplinary care team meetings and collaborate with Registered Nurse Care Managers (RNCMs) and clinical staff
- Facilitate in-home telehealth visits and help patients access educational resources
Required qualifications: High school diploma or equivalent, 1+ years case/care management experience (preferably across multiple settings), 2+ years healthcare experience, and experience with Medicare, Medicaid, or Special Needs populations. Physician office experience is required.
Preferred qualifications include Medical Assistant, LPN, Patient Care Technician, Community Health Worker credentials, or renal patient experience. Bilingual fluency in Spanish, Cantonese, or Mandarin is highly valued. BLS certification is required.
