Somatus is a leading outcomes-driven care provider supporting individuals with chronic conditions, particularly those with cardio, kidney, metabolic, and other chronic illnesses. The Patient Health Advocate (PHA) role is a hybrid position based in Charleston, SC, supporting high-needs Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) populations.
You will serve as the primary Somatus representative to physician practices and patients, establishing trust and building collaborative relationships. The role combines remote work with in-person visits to members' homes and physician offices. You'll work under the guidance of practice physicians and nurse care managers to coordinate integrated, interdisciplinary care.
Key responsibilities include: conducting patient outreach (telephonic and in-person) to introduce the Somatus program and encourage enrollment; assisting patients during care transitions to minimize readmissions; scheduling appointments and facilitating follow-up post-discharge care; documenting all activities in the care coordination platform; addressing language and cultural barriers to care management; coaching patients on self-care and clinical goals; helping patients access community and government resources; arranging transportation; and supporting telehealth visits.
You'll also prepare and manage monthly care team meetings, serve as the primary contact for provider practices regarding patient needs, educate practices about Somatus workflows, and use motivational interviewing techniques to encourage behavioral change.
Required qualifications: high school diploma or equivalent; 1+ years case/care management experience, preferably across multiple settings; 2+ years healthcare experience; experience with Medicare, Medicaid, or Special Needs populations; and physician office experience. Preferred: Medical Assistant, LPN, Patient Care Technician, Community Health Worker background, or renal patient exposure. Bilingual capability (Spanish, Cantonese, or Mandarin) is preferred. You must obtain BLS certification and demonstrate strong communication, cultural awareness, computer proficiency (MS Office, telecom), and ability to work effectively in multidisciplinary teams.
