Somatus is a leading outcomes-driven care provider supporting individuals and communities with chronic conditions, particularly cardio, kidney, metabolic, and other chronic diseases. The Patient Health Advocate (PHA) role is a hybrid position based in Fort Worth, combining remote work with in-person visits to physician offices and patient homes. You will serve as the primary Somatus representative to physician practices and patients, focusing on high-needs Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) populations. Your core responsibilities include establishing trust and building collaborative relationships with partnered physicians and practices, conducting patient outreach via phone and in-person to introduce the Somatus program and encourage enrollment, and serving as the primary contact for provider practices regarding patient needs and care coordination. Key duties encompass supporting integrated care team meetings, educating provider practices about Somatus workflows, functioning as a patient advocate throughout their care journey, and assisting with transitions of care to minimize avoidable readmissions. You will help patients schedule appointments, access community resources, arrange transportation, and address language and cultural barriers to care. You'll also support telehealth visits, use motivational interviewing techniques to encourage behavioral change, and document all activities in the care coordination platform. Required qualifications include a high school diploma, 1+ years of case/care management experience (preferably across multiple settings), and 2+ years of healthcare-related experience. Experience with Medicare, Medicaid, and/or Special Needs populations, as well as physician office work, is required. Preferred qualifications include medical assistant, LPN, patient care technician, community health worker credentials, or renal patient exposure. Bilingual fluency in Spanish, Cantonese, or Mandarin is preferred. You must demonstrate strong multi-disciplinary teamwork, excellent written and verbal communication with cultural awareness, proficiency in MS Office and telecom devices, and ability to obtain BLS certification.
