Somatus is a leading outcomes-driven care provider supporting individuals and communities with chronic conditions, particularly focusing on cardio, kidney, metabolic, and other chronic disease management. The Patient Health Advocate (PHA) role is a hybrid position based in El Paso, combining remote work with in-person visits to physician offices and patient homes. You will serve as the primary Somatus representative within partnered physician practices, responsible for establishing trust and building collaborative relationships with physicians, practice staff, and patients. Working under the guidance of practice physicians and nurse care managers, you'll support high-needs Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) populations facing multiple barriers to care access and treatment adherence. Key responsibilities include: conducting patient outreach (telephonic and in-person) to introduce the Somatus program and encourage enrollment; assisting patients during care transitions to minimize avoidable readmissions; scheduling appointments and accompanying patients when needed; addressing language and cultural barriers to care; coaching patients on self-care and behavioral changes using motivational interviewing techniques; helping patients access community and government-based services; facilitating care coordination platform documentation; supporting interdisciplinary care team meetings; and reinforcing clinical education about symptom response and preventive monitoring. You'll work closely with Registered Nurse Care Managers (RNCMs) and support telehealth visits, helping bridge gaps between patients and their healthcare providers. The role requires strong communication skills, cultural awareness, computer proficiency (MS Office, telecom platforms), and the ability to navigate complex healthcare workflows while maintaining focus on patient advocacy and improved clinical outcomes. Required qualifications: high school diploma or equivalent, 1+ years case/care management experience (preferably across multiple settings), 2+ years healthcare experience, and experience with Medicare, Medicaid, or Special Needs populations. Preferred qualifications include Medical Assistant, LPN, Patient Care Technician, or Community Health Worker credentials, and bilingual fluency in Spanish, Cantonese, or Mandarin for clinical conversations. BLS certification required.
