Salary: USD 100,000 - 140,000 / annual

Conduit Health is transforming post-acute care by building the first vertically integrated, AI-powered platform that unites ordering, telehealth, prescriptions, insurance, and fulfillment for medical equipment and supplies delivery to patients' homes. As Implementation Manager, you'll lead new implementations—state expansions, payor launches, and cross-functional initiatives—reporting to the Head of Operations. Sitting on the DME Operations team, you own project management of these launches and serve as connective tissue across Clinical Ops, Support, Payor Development, and Product. You'll gather deliverables, keep commitments on track, and fill the gap between strategy and delivery, ensuring new initiatives actually land rather than stalling in handoffs. Key responsibilities include: owning new state expansions from post-diligence through go-live; leading payor implementations and launches by coordinating eligibility, credentialing, billing, and fulfillment readiness; acting as connective tissue across M&A, Clinical Ops, Support, Payor Development, and Product to chase commitments and unblock handoffs; building source-of-truth launch and expansion trackers; running kick-offs and surfacing risks early; potentially owning network contracting admin; and reporting against launch KPIs to build repeatable playbooks. You bring organization and structure-building as a superpower, turning ambiguity into clear plans and detailed punch lists. You have proven project management experience taking initiatives from 0 to 1, strong stakeholder management skills, and expert-level communication tailored to different audiences. You excel at attention to detail, holding many moving parts simultaneously, and using systems, tools, and metrics to improve processes. Bonus experience includes management consulting or structured problem-solving background, healthcare/DME/health-tech experience, high-growth startup or scaling environment exposure, and M&A integration or post-acquisition operations familiarity.
