The Community Patient Engagement Specialist plays a critical role in driving growth by managing new member engagement and enrollment. This hybrid position splits time between remote work (2-3 days/week) and in-person community engagement (2-3 days/week) across hospitals, post-acute facilities, rehabilitation centers, and other healthcare facilities throughout the Chicagoland area.
Key responsibilities include conducting outreach via phone and in-person to educate prospective patients about program benefits, pre-screening patients for eligibility, answering questions and addressing concerns, and guiding them through the enrollment process. The role involves building trusting relationships with patients and caregivers to encourage program participation, maintaining documentation in the Karoo Health EHR system for tracking and reporting, and coordinating with hospital/facility case management and discharge planning teams. The specialist will participate in member engagement efforts across multiple channels including in-person visits, calls, texts, and email campaigns, ensuring patients understand program services and benefits.
