Habitat Health is a mission-driven healthcare organization expanding a scalable PACE (Program of All-Inclusive Care for the Elderly) model to serve older adults with comprehensive medical care and daily support services. The Clinical Services Manager is a key leadership role within the PACE Center, reporting to the Center Executive Director and responsible for overseeing nursing and clinic teams to ensure high-quality, participant-centered care.
Key responsibilities include providing clinical leadership and oversight to the Interdisciplinary Team (IDT) and clinic staff; ensuring clinical competency of direct reports through skills checks, coaching, mentoring, and performance management; leading onboarding, training, and continuing education for nursing and clinic team members to maintain high standards of care and regulatory compliance; monitoring adherence to clinic and IDT service level agreements, regulatory timelines, and documentation requirements; overseeing care transitions across settings (hospital, skilled nursing facility, home, etc.) in coordination with the Medical Director and center leaders; and driving achievement of key performance indicators through effective planning, workflow optimization, and participation in weekly tactical and monthly operating reviews.
The role partners closely with the Center Manager/Director to ensure overall center success. Depending on center census and structure, the Clinical Services Manager may oversee additional members of the IDT. The position requires maintaining compliance with PACE, DHCS, and CMS regulations.
