The Case Manager, RN is a clinical role that directly guides participant care as a key member of the interdisciplinary team (IDT). Reporting to clinical leadership, you will deliver and oversee all aspects of care coordination for seniors in the PACE program.
Day-to-day responsibilities include:
- Conducting clinic nursing assessments to determine participant needs, preferences, and goals
- Actively participating in IDT meetings to develop personalized participant care plans
- Delivering and documenting nursing interventions, including maintaining safe environments, responding to physician orders, and administering medications
- Collaborating with the WelbeHealth PACE Physician and other IDT members to coordinate care
- Communicating with participants and families to revise and adjust care plans
- Instructing participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies
- Visiting participants in their living environments for routine follow-ups and necessary nursing care
This role offers a Monday-through-Friday schedule with no weekends or on-call requirements, allowing you to build meaningful long-term relationships with participants rather than providing short-term episodic care. Requirements:
- Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
- Unencumbered New Jersey RN License
- Current Basic Life Support (BLS) and First Aid Certification required
- Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
- Ideal candidates have experience working with frail and elderly populations
