The Case Manager, RN is a clinical role that directly guides participant care as a key member of the interdisciplinary team (IDT).
Reporting to clinical leadership, you will deliver and oversee all aspects of care coordination for vulnerable seniors. Day-to-day responsibilities include conducting required PACE assessments, developing and implementing personalized participant care plans, and supporting transitions of care. You will conduct clinic nursing assessments to determine participant needs, preferences, and goals, and actively participate in IDT meetings to develop care plans. You will deliver and document nursing interventions as outlined in care plans, including maintaining safe environments, responding to physician orders, and administering medications. Collaboration is central to the role—you will work closely with the WelbeHealth PACE Physician and other IDT members to coordinate care, and communicate with participants and families to revise care plans as needed. You will also educate participants, families, and caregivers about disease processes, self-care techniques, and prevention strategies.
This role offers a consistent Monday-through-Friday schedule with no weekends or on-call requirements, allowing you to build meaningful long-term relationships with participants rather than providing short-term care. REQUIREMENTS:
- Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
- Unencumbered California RN License (required)
- Current Basic Life Support (BLS) and First Aid Certification (required)
- Two (2) years of nursing experience in a clinical setting with a frail or elderly population (preferred)
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
- COVID-19 vaccination required prior to employment
