Salary: USD 120,000 - 150,000 / annual

As Care Team Manager for San Diego, you will lead daily clinical operations and build a high-performing team of Registered Nurses, Nurse Practitioners, LICSWs, Health Partners, and administrative staff. You will manage team members, oversee schedules, develop and enforce policies and procedures, and ensure smooth care delivery across virtual, in-person, and at-home settings.
Key responsibilities include:
**Clinical Operations Leadership**: Lead day-to-day clinical operations including staffing and scheduling; build and sustain a high-performing team culture; hire, onboard, coach, and develop clinical and administrative staff; partner with the Market Operations Leader and Medical Director on strategy and operations.
**Data-Driven Execution**: Monitor performance using operational and clinical data; set clear performance metrics across quality, patient experience, productivity, and efficiency; foster continuous improvement culture using data to identify trends and test new approaches.
**Care Delivery Oversight**: Ensure care delivery across virtual, in-person, and at-home settings is efficient and high quality; implement strategies to improve access and quality while optimizing team time; handle escalated patient issues and drive service recovery.
**Partnerships & Market Development**: Serve as trusted partner to San Diego providers and market stakeholders, supporting Daymark's visibility and growth.
**Cross-Functional Collaboration**: Work closely with Enrollment, Clinical Operations, Tech, Marketing, and Growth teams to align Care Team efforts with organizational goals.
**Requirements:**
- 5+ years of experience in healthcare operations, care management, or related clinical leadership role with prior people management experience
- Strong background in patient care delivery and clinical operations; experience with virtual care and/or at-home healthcare delivery models is a plus
- Clinical background (RN, NP, LICSW, or equivalent licensure) is a plus
- Experience hiring, onboarding, and developing both clinical and administrative staff in fast-paced environments
- Deep commitment to delivering exceptional quality of care and support for patients and families; understanding of complex care models strongly valued
- Strong operational acumen including scheduling, staffing, and budget management
- Data-driven mindset with ability to monitor clinical outcomes, track team performance metrics, and use data to drive continuous improvement
- Excellent interpersonal and communication skills; ability to build trust with clinical staff, community partners, and patients/families
- Comfort with ambiguity and startup environments; ability to build and iterate on processes while maintaining high standards of care delivery
