Strive Health is transforming chronic disease management by identifying risk earlier, coordinating thoughtful care, and supporting patients through their health journey. The Patient Navigator role supports Strive's care team in keeping patients engaged with a comprehensive care program specifically designed for people with kidney conditions.
You will be responsible for proactively reaching out to both current and prospective patients—primarily through phone calls and face-to-face interactions—to educate them about available care services and enroll them within an assigned geographic area. While most patient interactions occur by phone, you will have opportunities to meet patients in person at office visits or in their homes.
Key responsibilities include:
- Conducting outbound phone outreach and face-to-face activities to educate and enroll patients in the kidney care program
- Building strong relationships with patients to keep them engaged and scheduled for program appointments
- Serving as an educational resource and helping patients navigate questions about their care
- Documenting patient interactions and program enrollment in the electronic health record system
- Collaborating with the care team to support patient success
- Maintaining accurate records and meeting program metrics
Success is measured by your ability to schedule patients for program appointments through relationship-building, education, and navigation support. The role offers hybrid flexibility—work from home while fulfilling in-person needs at the office, clinic, or patient homes.
Qualifications:
- High school diploma or equivalent required
- Experience with promoting and selling primary care or other healthcare-related services to high-needs populations preferred
- Experience in healthcare, specifically educating patients on care models or insurance plans preferred
- Community health experience preferred
- Empathy for the burden of kidney disease
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Comfort with phone-based outreach and face-to-face patient engagement
