Strive Health is seeking a Patient Navigator to support its care team in keeping patients engaged with a comprehensive care program designed for people with kidney conditions. This role is hybrid, combining remote work with in-person patient interactions via phone, office visits, and home visits.
Key Responsibilities:
- Proactively outreach to current and prospective patients through face-to-face activities and outbound phone calling to educate them on available care services and enroll them within an assigned geographic area.
- Build strong relationships with patients to drive engagement and program participation.
- Serve as an educational resource, helping patients understand their care options and navigate questions about kidney disease management.
- Schedule patients for program appointments and track enrollment metrics.
- Collaborate with the care team to ensure patients remain engaged throughout their health journey.
- Document patient interactions and maintain accurate records in the system.
- Identify and address barriers to patient engagement and program participation.
