Foundation Health is an AI operating system for pharmacy, purpose-built to streamline care coordination and operational workflows in specialty pharmacy. The company is in production with enterprise health systems across the US and works directly with major pharmacy partners. The platform tackles prior authorizations, paperwork delays, and care coordination gaps using AI agents and deep healthcare integrations. As a Staff Engineer, you will own the technical direction of core product areas including prior auth pipelines, Epic integrations, and AI agents handling patient communications. You'll make architectural decisions, write substantial code yourself, and set technical standards for the team. As one of the first senior engineers in the Orlando hub, you'll also shape team culture and working practices. Key technical responsibilities include: - Building production agentic systems in a regulated healthcare environment with automated workflows, LLM-assisted QA, and AI features (RAG, function calling, evals) where output directly impacts patient care - Multi-component system design across product domains covering Epic integration, prior auth pipelines, and patient communication infrastructure - Setting the technical bar through your own code and architectural decisions; your PRs should be learning resources for others - Delivery accountability across workstreams with cross-team dependencies, product roadmaps, and shipping software that solves real user problems The role is hybrid, 3 days per week in the downtown Orlando office. The company operates with minimal legacy systems, no change board, and fast shipping cycles. You'll work closely with the VP of Engineering who is based in the same office. Compensation includes equity (early-stage, meaningful stake), and Orlando's cost of living and lack of state income tax make the package competitive. The company is backed by Define Ventures and led by a 2x founder; the VP of Engineering previously worked at DeepMind. REQUIREMENTS: - Track record of building complex systems from scratch with ability to articulate specific technical decisions and rationale - High agency and ownership mindset; you identify what needs to happen and execute without waiting for perfectly scoped tickets - Comfort with ambiguity in a fast-moving, rapidly changing environment - Daily use of AI tools (team is Claude/Cursor-native) and hunger to push AI capabilities further - Experience with production systems, ideally in regulated or high-stakes environments - Familiarity with healthcare integrations (Epic) or agentic AI systems is a plus
