Salary: USD 180,000 - 250,000 / annual

HeartFlow is a publicly traded medical technology company (HTFL) that develops AI-driven diagnostic solutions for coronary artery disease, the leading cause of death worldwide. The company's flagship product, HeartFlow FFR_CT Analysis, uses AI to generate 3D models of coronary arteries and assess blood flow impact, helping clinicians identify stenoses and characterize atherosclerosis. The platform has been used for over 750,000 patients globally and is cleared in the US, UK, Europe, Japan, and Canada. We are seeking a Senior or Staff Software Engineer to design, build, and operate the backend services powering our clinical platform. You will own the systems that move patient imaging data through our AI analysis pipeline and deliver results to clinicians worldwide. This role emphasizes end-to-end ownership from design through production, with a high bar for correctness and security—patient safety is paramount. Key Responsibilities: - Design, build, test, and operate backend services and APIs for the clinical platform, owning work from design review through production monitoring - Translate product and clinical requirements into clear technical designs; write and review design documents with explicit trade-off analysis (correctness, performance, security, maintainability) - Write high-quality, well-tested code and maintain rigorous code review standards for correctness, readability, and regulated-environment documentation - Build and improve services and data pipelines that reliably and securely move patient imaging data and analysis results at scale - Implement secure-by-default patterns for authentication, authorization, access control, and data integrity aligned with platform security architecture - Own reliability and performance: instrument systems, set up monitoring and alerting, investigate production issues, and participate in on-call rotation - Contribute to CI/CD pipelines, infrastructure-as-code, and developer tooling to accelerate team velocity - Produce design, verification, and traceability documentation required for FDA-regulated medical software; build to HIPAA standards for patient data handling - Mentor engineers through code review, pairing, and design feedback; help onboard new team members and raise engineering standards - Use AI coding assistants and agents in production work with clear judgment about effectiveness and verification - Partner with Product, Clinical, Operations, and Regulatory teams to scope work and communicate technical trade-offs to non-engineering stakeholders - (Staff level) Lead technical initiatives spanning multiple services and teams—drive design, align stakeholders, break work into milestones, and carry through to production - (Staff level) Identify technical risk and dependencies early; help define and evolve engineering best practices, tooling standards, and architecture patterns across teams Qualifications Required: - Bachelor's degree in Computer Science, Engineering, or related discipline, or equivalent professional experience - 7+ years of professional software engineering experience (Senior level); 10+ years including demonstrated technical leadership beyond a single team (Staff level) - Strong backend programming skills in Python or Java with solid fundamentals in object-oriented programming, data modeling, concurrency, and API design - Hands-on experience building and operating services on AWS; working knowledge of both SQL and NoSQL data stores - Demonstrated experience designing, shipping, and operating production services in distributed or microservices architecture - Track record of owning a meaningful service or component end-to-end, including reliability and performance in production - Disciplined approach to automated testing, code review, and debugging; ability to explain correctness, not just functionality - Experience with Agile development methodologies and project management tools - Working knowledge of web and OWASP security best practices - Hands-on experience with AWS services (Lambda, ECS, EKS, Fargate, S3, SQS, VPC), infrastructure-as-code tooling (Terraform, CDK), CI/CD, and observability - Ability to write and speak clearly about technical work to both engineering and non-engineering stakeholders - Demonstrated ability to work in fast-paced, collaborative environments Desirable Experience: - Medical imaging data (DICOM), healthcare interoperability standards (HL7, FHIR), or systems handling PHI - Deep production experience with Django and Django ORM, including migration strategy and query performance tuning on large datasets - Experience in controlled development processes—FDA-regulated medical software or other regulated/safety-critical domains - (Staff candidates) Track record as tech lead or staff engineer driving technical initiatives across multiple teams
